Our Medical Management team includes a variety of healthcare professionals who work together to remove barriers, making it easier for Members to manage their care and obtain durable medical equipment.

Complex care managers

When it comes to serious illness, our nationally accredited complex care management programs provide compassionate, personalized support for metastatic cancers, pediatric intensive care and transplants. We can help to coordinate care between providers, in-patient facilities and national transplant networks. 

Preventing hospital readmission

With a 69% reduction in readmission rate, we work hard to help you get well. In-house specialists coordinate with our Care Management team to help Members facing a high risk of readmission, including partnering with home health agencies, community agency care teams and other local agencies. 

Infusion Site of Care Program

Our voluntary Infusion Site of Care Program can save on your medical costs by allowing you to transition certain intravenous (IV) medications and infusions to a preferred site of care, including your own home.

We work closely to help those seeking treatment for substance use disorder. Our team helps to facilitate access to high-quality, cost-effective and convenient in-network program options. This includes transitional support after discharge from an inpatient behavioral health or substance use facility.

What would I call Member Services for?

Our Maine-based Member Services team is here and happy to assist you. They can help with a variety of questions you may have, including:

  • Questions about your benefits and coverage
  • Setting up and navigating through your Member portal
  • Enrolling in autopay
  • Finding providers and facilities 

We also have dedicated Claim Service Associates who are here to help with questions you may have regarding claims.

What is a Care Manager? How can someone help me?

Think of our Care Managers as a case manager assigned to help coordinate your care. From finding resources like a ride home from the hospital to organizing your continuity of care paperwork so you never miss a step, Community Health Options’ Care Managers can help you navigate your most difficult health care issues.

The best part? This service is available to all Members at no added cost. You can access uninterrupted care no matter what you’re facing or what plan you’re on—that’s the Community Health Options way.

I got a message from a company I don’t recognize — are they working with Community Health Options?

Most likely! We partner with select vendors to provide our Members with a more personalized experience. You may be contacted by one of these companies as part of our custom outreach programs: 

  • Blue Circle is a partner who works with Members over the age of 18 with Type 1 diabetes. They will coordinate with a Member’s existing care team to provide free virtual endocrinology care, nutrition advice, peer support, counseling, financial assistance, access to insulin and education, including how to use glucose monitoring tools and insulin pumps through phone, text and video calls. You can learn more about this program from our recent news release.

  • Twig is a partner who works with members with Type 2 diabetes. Twig Health’s licensed nurses will text and call Members, offering help accessing diabetes-related care, including scheduling A1C and blood pressure tests, foot exams, nutrition counseling, eye exams and other need. If you receive a message from (207) 401-6402, this is not spam and is verified outreach. 

  • UST is a partner who encourages Members to schedule primary care visits.

  • Signify is a partner who schedules and arranges in-home annual wellness visits with Members.
